Job Description:Job Profile Summary
Is responsible for caring for vulnerable patients in their home or other community settings to provide health promotion, disease management, care transitions, education and other services as needed. Will work alongside emergency medical services, emergency department, and primary care physicians in both the clinic and hospital setting to identify patients who could benefit from community paramedic services. The community paramedic will work under the license of the medical director utilizing established written protocols in conjunction with the patient’s primary care physician and the patient’s established care plan.
