Location/Schedule: This position is based out of our Garlington Health Center in NE Portland, but work with clients will be performed primarily in the community and in participant’s homes. The schedule for this position is Monday through Friday, 8:30 a.m. to 5:00 p.m.

Position Description
The Community Program Supervisor II is responsible for collaborative program operation of Cascadia’s Housing Outreach Team (HOT), the supervision of a portion of HOT’s case managers and housing specialists, as well as a partial caseload, while supporting a Trauma Informed Care environment. This position organizes, motivates, coaches, trains and supports Housing Outreach Team staff by providing organizational structure, clinical supervision, mentorship of team members, in the field leadership, philosophical direction and serves as the liaison with Cascadia’s management. With the high prevalence of trauma among individuals receiving mental health services, it is required the Program Supervisor understand the effects of trauma on health, coping, and other aspects of the lives of those we serve. This position involves participation in the development and implementation of program goals and objectives to promote a strengths-based, client centered approach in supervision of staff to support them in their work with supportive housing program participants. The Program Supervisor assists the Program Managers in the implementation and administration of multiple grants and funding sources, including HUD McKinney grants, City of Portland funding, State of Oregon rental assistance funding and Medicaid billing.

Essential Duties

Provide clinical leadership, including integration of clinical best practices into day-to-day program activities, including but not limited to Housing First, Harm Reduction, Trauma Informed care, Motivational Interviewing, and Recovery Model.Provide training, guidance, coaching and supervision to staff to ensure that program goals and objectives are met.Establish work plans and priorities for staff members and assess the progress of staff members; take corrective action as needed. Conduct performance reviews for staff members or provide the Program Manager with information necessary to assess performance.Document and maintain electronic health record (EHR) and ancillary records in a manner that assures compliance with all agency policies, program procedures and local, state, and federal regulations. Maintain accurate and up-to-date documentation associated with billing, assessment, treatment, client’s progress and other activities, as required.Ensure that staff members maintain accurate and up-to-date documentation as required associated with billing, assessment, treatment, clients’ progress and other activities. Ensure that staff members complete electronic medical records (EMR) as required using Cascadia software.Complete employee timecards on a semi-monthly basis and track staff attendance.Track mental health assessment and treatment plan due dates and completion; support staff in developing processes for tracking and scheduling assessment and treatment planning appointments with clients.Monitor staff service hour (productivity) goals and provide coaching to help staff achieve or exceed service hour expectations; address issues staff may have in meeting service hour standards and in maintaining contact with clients.Under the direction of the Program Manager, develop programming for various housing sites; model therapeutic interventions and counseling with program clients and assist staff members by helping to resolve difficult client/program situations.Provide individual and/or group therapeutic training for staff to assist participants in learning or increasing practical living and coping skills. Collaborate with Program Manager to evaluate and improve site/grant programming, including group services, throughout the HOT program. Participate in regular supervision. Attend and lead, when directed, clinical case conferences and staff meetings to gain/share knowledge and communicate program and participant needs.Collaborate with Program Manager in the hiring of new staff including taking the lead to post positions and complete new hire paperwork.Develop effective and friendly relationships with partner programs, property management partners, and funders. Proactively communicate with partner organizations and/or housing providers to address concerns and support staff in working collaboratively with partners.Prepare various reports for the Program Manager or other stakeholders as required/requested.Monitor expenditures and use of agency/program resources, making suggestions and putting plans into action to enhance efficiencies and improve quality of services and operations.Develop staffing plan and coverage plan to cover for absences and/or position vacancies.Track new referrals coming into the program, including meeting with new referrals when an opening occurs and develop plan of engagement and program enrollment.Assist in housing grant audits and take the lead in tracking mental health compliance audit completion with the team.Understand different funding sources within HOT program and approve requests for payments in light of funding requirements and funding availability.Respect and honor client rights and responsibilities and demonstrate professional boundaries and ethics.Assist in crisis situations following emergency protocols/procedures and coordinating with the treatment team with interventions which support a trauma informed environment for participants and staff. Demonstrate responsibility for safety of consumers, staff and property; demonstrate familiarity with fire regulations and evacuation procedures.Adhere to mandatory abuse reporting laws and HIPAA requirements.Provide input on all relevant documentation and procedural requirements.Drive personal car and agency vehicles to meet with clients in their homes and in the community.Maintain personal schedule including setting up individual appointments with clients, staff, and partners. Provide outreach to clients if clients become less engaged including providing support to staff who are working to reengage clients.Work cooperatively in a team setting to coordinate continuity of care.Provide leadership, complete essential trainings, keep informed and inform staff of new rules, requirements and expectations related to insurance, compliance standards, outpatient OARS, HUD McKinney rules and regulations, and local, state and federal housing laws including Fair Housing and Landlord-Tenant Law.Complete annual employee training requirements on a timely basis as defined by the agency and program. Attend seminars, training and other educational opportunities in order to develop professional skills and abilities.Participate in all scheduled staff meetings, supervision sessions, and other departmental and agency meetings including Housing Supports & Services Leadership meetings and Garlington Clinic Leadership meetings.Perform other duties as assigned.Qualifications
Must meet the requirements to be credentialed as a Qualified Mental Health Professional (QMHP) through MHACBO. Must have a Master’s Degree in a behavioral science field from an accredited college or university. Demonstrate the competencies to become credentialed as a QMHP as outlined in the Oregon Administrative Rules (OAR). Certification must be current and in compliance with the OAR’s.The CADC credential is preferred in addition to a Master’s Degree given the high occurrence of substance use/abuse.A minimum of three years of clinical experience providing inpatient or outpatient services is preferred.Experience working with individuals with the experience of homelessness and with individuals experiencing substance use issues is preferred.Conduct a Mental Status assessment and formulate a diagnosis according to the most current DSMUnderstand recovery model, harm reduction model, motivational interviewing, illness management and recovery, evidence-based practices, and strength-based case management philosophies and strategiesOperate computer hardware and software at a level needed to effectively perform job functionsEnter data and retrieve dataPrepare reportsPossess a valid driver’s license, three-year driving history; clean driving record, ability to pass Driver Motor Vehicle (DMV) check and complete the Cascadia online Defensive Driving course.Able to perform CPR, First Aid, and properly use AED equipment. CPR/ First Aid certifications are required and must remain current.Working Conditions
Environmental: Work with clients is performed primarily in the community and in participant’s homes with some office work being performed at the Garlington Clinic where the Housing Outreach Team is based. The majority of the work is performed during normal business hours; however occasional weekday evenings or a Saturday or Sunday may be necessary. Employee will carry Cascadia provided cell phones and laptop computers to assist in completing their work in the field. Daily routines may be hectic and will likely require the Program Supervisor to perform various functions and, on occasion, perform the tasks of other employees as needed. Meal periods and breaks are subject to interruption. Universal Precautions and remaining alert to the environment are critical to address the potential for violent behavior, exposure to disease, biohazards, noise and contaminants.
Mental Demands: The work assigned is diverse and involves addressing new and unusual circumstances in which outcomes may negatively affect costs, employee morale or clients. The work regularly involves a degree of unpredictability and disruption of planned tasks requiring a flexible time management approach. There will be need for a high degree of time management, crisis triage/response, and prioritizing skills, due to fluctuating programmatic and staffing routines.
Physical Demands: Activities and structure support community integration assisting clients in continued growth for successful independent living. Employee must be approved to provide transportation using agency vehicles in order to support resident access to the community. The Program Supervisor must have own car and be able to use it to travel to home visits and community appointments. Employee must also have the ability to assist residents to participate in a variety of activities, events, and excursions involving skills building and community integration. The Program Supervisor must have the ability to assist the team in providing safety to all residents and staff in potentially volatile situations. When the need arises, staff may be asked to perform CPR and respond appropriately to crisis situations using approved de-escalation techniques to minimize risk to clients, residents, staff and the larger community.
