and links appropriate services advocates for and monitors client needs to ensure the provision of comprehensive services to meet identified client needs Coordinates and provides services in an integrated Individualized Recovery Plan IRP Ongoing interaction with individuals who have been diagnosed with severe and chronic mental illness andor substance abuse diagnoses All employees in this
