As a Community Service Provider, you will provide home and community-based services to help children with emotional and mental health challenges. You will travel locally to work with children and families in their communities providing a core of rehabilitative services to ensure success, including non-licensed counseling and support for youth and family, skill building, peer support, caregiver support and advocacy, pre-vocational services and respite. You will report to a Community Service Provider Supervisor, and work onsite within the community, as travel is required and documentation may be completed remotely.
Responsibilities:
Provide services within a Family-driven Care framework, including respecting family choice and voice, believing in each family’s potential to learn and grow, and utilizing strategies that empower families Work with family members to identify goals and objectives of treatment Provide services based on the needs as identified in the Treatment Plan Coordinate with the child and caregiver on the scheduling and provision of services Participate in relevant treatment meetings and attend all required trainings Complete all documentationRequirements and Education:
Bachelor's degree in a related field and at least two years of related experience OR Master's degree in Social Work, counseling, psych, or a related human service and at least one year of related experience Valid NYS Driver's License requiredBenefits
