The Arc Erie County is currently seeking Community Services Direct Support Professionals (DSPs) who wish to get paid to have fun! Our Community Services DSP II’s provide direct supervision to community & residential individuals in all facets of daily living (one-on-one or group settings). Full Job Description is listed below: Job Description Job Title: Community Habilitation DSP Department: VES Job Grade: 2 Position Summary: Community Habilitation DSPs provide teaching opportunities to individuals which include but are not limited to activities of daily living, self-care skills, money management, cooking, community integration, travel, training and communication. These services are tailored to the individual’s specific skills and abilities. Services are provided in the individual’s home and in the community and maybe 1-1 or in groups. Schedules/assignments/locations are subject to change due to the needs of the program, and to assist staff with maintaining their scheduled hours. Reporting Relationship: Reports directly to the Community Programs Manager. Job Duties and Responsibilities: + Supports people by providing person-centered assistance and training in areas such as, but not limited to activities of daily living, self-care skills, money management, cooking, travel training and communication. + Assures peoples overall safety through maintaining safety standards, familiarity with emergency safety procedures and providing regular training to people in these areas. + Transports people to and participates in recreational activities as applicable to the individuals Staff Action Plan. + Assists people with all personal shopping needs. + Assists people with personal monies. + Documents the provision of services, skill acquisitions and supports daily, contemporaneous and consistent with the Staff Action Plan and Agency procedures. + Use of EVV (Electronic Visit Verification) during service provision in order to report information related to each visit. + Prepares or assists people with meal preparation, meal planning and purchasing of foods. + Maintains a clean, safe environment by providing training and assistance to people in the areas of housekeeping and minor maintenance. + Provides input to the treatment team in the development of a Staff Action Plan. + Assists in the assessment of a person’s skills and behavior. + Implements and documents progress of Staff Action Plan. + Attends regular staff meetings, trainings, supervisory conferences and any other assigned meetings. + Checks agency email regularly, updates supervisor on changes in contact information, maintains communication. + Assists people with community integration, by providing support and training. + Assures confidentiality of information about people. + Interacts with people’s families, other collaterals and the community at large as appropriate. + Attendance at agency required trainings, as well as Relias online trainings must be taken within 90 days of hire, and quarterly as assigned. + The expectation of this position is that staff will remain awake and perform duties as per the job description or as assigned. + Other duties as assigned by supervisor. Qualifications and Education Requirements: Associate’s Degree PLUS six months experience in Human Service Field OR High School or equivalent diploma PLUS one year of experience in the Human Services field preferred. PLUS Have and maintain a valid NYS driver's license and approved driving record. Ownership and access to an insured car in good operating condition is required. Receive supervision instruction in a positive manner. Adhere to and administer Agency’s policies, procedures, rules and regulations. Maintain a professional attitude and appearance at all times. + Equipment Operation: Good knowledge and ability in the operation of motor vehicles; Ability to use office equipment, such as, computer, copier, fax machine, etc. Ability to come in contact with normal household dust/dirt.
