Position: Community Support Worker
Location: Galway Aspergers Outreach Support, Co. Galway
Fixed Term Contract 12 months, part time 25 hours
Job Purpose
Working in partnership with the HSE and the Asperger Action Group, the post holder will be responsible for the delivery of a quality client focused service to people with Asperger Syndrome. The post holder will have a role in the planning, development, delivery, and ongoing review of the service. This will include formal and informal liaison and collaboration with key stakeholders – the clients, their family and friends, the Steering Group, and Multi Disciplinary professionals where appropriate.
Of equal importance will be the development of a network of community resources and facilities to ensure that the project participants have full access to all aspects of community life, especially housing, educational, training, employment, leisure, and social opportunities. A key objective is that personal plans and activities including meaningful occupational activity, such as education and employment are sustainable. This will require the post holder to work flexibly, and will include evening and weekend activity.
Minimum Education & Skills required
A third level graduate, Health & Social Care or equivalent. Minimum of 1-2 years supervisory experience in Autism specific services. Experience of the design and delivery of individual programmes for people with disabilities or those who are marginalised. Previous experience of working 1:1 in the Community or in person’s homes Understand and knowledge of Asperger’s and or mental health disability. Irish speaker is highly desirable.
Job Duties & Responsibilities
Supporting service users to identify the community based supports they would like to avail of and work with service users to identify practical solutions to any barriers to accessing these supports. To advocate on behalf of and support self-advocacy for service users in a number of settings, including, with family, HSE, community groups etc. Research and devise Community Profiles, sourcing activities in service user’s communities. Facilitate the continual review of service user progress. To plan, develop, implement, monitor and review appropriate educational, recreational and occupational programs within the community ensuring best practice in line with up to date research regarding improving people Quality of Life. To liaise with families, the multidisciplinary team and key community based personnel in the pursuit of accessible community activities for service users. To enshrine the concept of independent living by supporting service users to integrate in to their local community and access local resources.
Additional requirements
Responsible for the operational management and ongoing development of services supporting people with complex needs. These are customer focused and cost efficient and operate within Person Centred Planning frameworks and processes. Responsibility to line manage a large team across a large geographical area. Responsibility to manage service budgets and ensure proper budgetary controls are in place. Full clean driving license. To comply with health and safety legislation. To maintain a basic knowledge of first aid/personal care and to implement good hygiene practices.
