Our team-based approach helps connect the dots for patients with complex challenges. Our team is looking for a board-eligible or board-certified Family Medicine, Internal Medicine, or Geriatrics physician to join our team. Our team consists of patient care navigator, nurse case manager, clinical pharmacist, physicians, advanced practice providers, social workers, clinical service specialists, and an in house chaplain. Care Connections sees patients in inpatient, outpatient, and home care settings. Our holistic approach to patient care allows us to focus on patient well-being and provider satisfaction. Our physicians enjoy a Monday through Friday dayshift schedule with minimal call shared within the group. The ideal candidate will be passionate about serving the underserved population of Lancaster and comfortable with handling complex cases.
