Continuing Care Coordinator
Under limited supervision, assists Care Management team with a variety of administrative functions. Coordinates with the Care Management team and other disciplines throughout the hospital to ensure smooth care transitions. Responsible for maintaining hospital compliance with a variety of documentation, including but not limited to MOLST forms, IM letters, etc.

Education:
Bachelor’s degree or equivalent experience preferred
Experience:
Three years’ experience in a health care setting preferred
Three years’ customer service experience required
Skills:
• Knowledgeable of available community resources
• Advanced skills in oral and written communication
• Skill in organizing workload and balancing many tasks
• Skills in time management
• Skills in data and statistical analysis
• Ability to evaluate data gathered for medical plan of treatment

Patient & Workplace Safety: 

Employee has knowledge and understanding of patient and workforce safety as it relates to job duties. 

Patient Population: 

Demonstrates competency in the delivery of care and applies the knowledge to meet age-specific needs if applicable. 

Pay Range

$18.86 - $28.30

Final salary offer will be based on the candidate's qualifications, education, experience and alignment with our organizational needs.
