Responsibilities

Provide coordination of patient care throughout the healthcare continuum

Provide coordination of communications with patients, families, physicians, and healthcare team members.Coordinate patient scheduling with appropriate resources or clinical services and maintains required documentation.Support standard patient access initiatives across the healthcare continuum.Provide support with navigating the healthcare system and assists patients with utilizing available community support services.
Requirements

Education:

High School diploma equivalency with 2 years of cumulative experience ORAssociate's degree/Technical degree OR4 years of applicable cumulative job specific experience required.
Additional Preferences

RESPONSIBILITIES WILL INCLUDE THE FOLLOWING:

Assist patients with navigating/coordinating their surgeries scheduled within Ascension St. Vincent EvansvilleWork indirectly with the VP of Finance & the Director of Provider Relations to ensure all patients have what they need for their surgeryNavigate multiple computer/data systems within the Ascension St. Vincent network to obtain information needed to adequately assist patients with schedulingWork on multiple projects simultaneously while maintaining the highest level of confidentialityCreate/Track information in GoogleSheetsFollow up on any outstanding items for patientsHave knowledge of community support systems available to Ascension St. Vincent Evansville to provide assistant to patients when neededPossess excellent communication skills when conversing with leadership and providers on all levels
