Coordinator, Drug Policy Center – Ambulatory Care
Major Responsibilities:Collects, evaluates, and reports on available information on medication use and appropriately applies it to optimize medication use policy decisions by the Pharmacy and Therapeutics Committee and others.Serves as a resource to pharmacy team staff and other health care professionals to provide expert consultation regarding medications.Recommends and implements strategies that minimize cost and optimize medication use outcomes.Designs, implements, and/or coordinates changes to improve the safety of medication use at Advocate Aurora Health Care.Researches and develops medication related findings and initiatives to other caregivers including medical and nursing staff, pharmacy, administration, and patients as appropriate.Effectively communicates medication related findings and initiatives to other caregivers, including medical and nursing staff, pharmacy, administration, and patients as appropriate.Develops, evaluates, and implements department activities to support quality outcome measures and external standards.Educates and trains pharmacy department caregivers, including pharmacists, residents, interns, and students.
Licensure, Registration, and/or Certification Required:Pharmacist license issued by the state in which the team member practices.
Education Required:Bachelor's Degree (or equivalent knowledge) in Pharmacy.
Experience Required:Typically requires 5 years of experience in pharmaceutical services, with experiences in formulary management and drug information.
Knowledge, Skills & Abilities Required:Specialty certification required (for external hires, certification must be obtained within one year of hire).Maintains advance skills in area of practice, demonstrating a commitment to continuing professional growth and advancement of the pharmacy profession.Proficiency in on-line literature retrieval and evidence-based medicine.Knowledge of formulary systems and Pharmacy and Therapeutics Committee processes.Current knowledge and understanding of medication safety trends and concerns.Demonstrated ability to work effectively with providers and other team members in collaborative decision making, team building, customer service and conflict resolution.Must have demonstrated abilities in working independently toward specific results, devising new methods, or adapting standard procedures to meet new conditions while making decisions based on precedent and/or organizational policy.
Physical Requirements and Working Conditions:Must have fine motor manipulation of hands and wrists to use computer keyboard.May be exposed to chemical hazards, so will be required to wear protective clothing as necessary.Must have functional speech, vision, and hearing.Must be able to sit, stand, and walk for prolonged periods.Exposed to normal office environment.This position requires travel so will be exposed to weather and road conditions.Operates all equipment necessary to perform the job.
