Overview Functions as a member of the cancer care delivery team in order to contribute to the developmental and coordination of the overall treatment plan. The goal is to assist patients and families facing the impact of a cancer diagnosis by helping them to identify social, emotional, and environmental needs and to connect them with available resources and services. Plans and implements programs and activities to support the goals of the cancer center and assesses program delivery and effectiveness. This position specifically works with malignant hematology patients undergoing stem cell transplant and/or cellular therapy. REQNUMBER: 32124
