As a Coordinator, Palliative Care Navigation with Vancouver Coastal Health you will:
Administer the palliative care registry and priority access to a comprehensive range of palliative care services by establishing and maintaining a single access system for clients diagnosed with a life-limiting illness and their families.Provide administrative support and professional advice to clients with palliative care needs.Facilitate regular discussions with clinicians in different settings to determine the most appropriate level of care for clients during different phases of their palliation and ensures their transition across the continuum of palliative care services.Determine priority placement and access of clients to all community hospice and acute palliative care beds in applicable location.Participate in the planning and development of an integrated palliative care program through directions provided by the applicable hospice-palliative program.Conduct public education sessions.Supervise and provide direction for clerical support staff including daily workload management.Ensure appropriate tools are developed for collection of relevant data and timely generation of required reports. QualificationsEducation & Experience
Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).Two (2) years' recent, related nursing experience including one year's related administrative and palliative care program development experience or an equivalent combination of education, training and experience.Advanced palliative care nursing certification (CHPCN(C))Knowledge, Skills & Abilities
Knowledge of clinical practice related to care management, interventions and care of the designated client populations and the ability to integrate this knowledge within the interprofessional team and community agencies.Knowledge and demonstrated skill in the provision of evidence-based clinical practice related to designated client population(s).Knowledge of other health care disciplines and their role in patient care.Knowledge of research process and methodology.Ability to supervise.Demonstrated ability to provide leadership and work direction.Demonstrated ability to work collaboratively as a member of an interdisciplinary team.Demonstrated ability to communicate orally and in writing.Demonstrated ability to communicate with, and deal effectively with, co-workers, physicians, other health care staff, patients and their families, and staff of outside agencies.Demonstrated ability to effectively deal with conflict situations.Demonstrated ability to adjust to new or unexpected events.Demonstrated ability to plan, organize and prioritize work.Demonstrated physical ability to perform the duties of the position.Demonstrated skill in the use of medical equipment and supplies appropriate to designated clinical area.Demonstrated skill in techniques appropriate to designated clinical area.Demonstrated skill in CPR techniques.Basic computer literacy to operate a computerized patient care information system and various software programs.Ability to operate related equipment.
