Coordinator-Valve RN - ME Advanced Disease Center
Summary The Valve Coordinator functions under the direction of the Administrative Director of Transplant Services. Participates in the clinical management of patients with cardiac valvular disease through the continuum of care in collaboration with the Baptist Heart Team. Coordinates the clinical evaluation of potential TAVR and TEER candidates. Participates extensively in the clinical management of patients referred to and treated through the Structural Heart Program. Provides ongoing education and support for the patient/caregiver through the entire process from referral to intervention to post-procedure care. Responsibilities + Receive and review referrals for appropriateness, reviewing with an attending provider as needed. + Establish contact with patient/family. Explain process and work-up and set expectations. + Obtain all outside imaging, scheduling screening procedures and valve clinic visit. + Assist patient/family with navigation of testing, valve clinic visit. + Participate in weekly Valve Conference with Heart Team. + Perform functional assessments and frailty testing. + Have all testing available for review by cardiologist and CTS at valve clinic visit. + Participate in development of treatment plan. + Patient and family education. + Facilitate direct contact between cardiologist and referring physician regarding treatment plan. + Coordinate any additional testing i.e. Cath, TEE. + Upload required data to data share site and maintain communication with the vendor. + Facilitate scheduling and pre-authorization of TAVR/TEER procedure. + Serves as a clinical expert and consultant to nursing, and team members at Baptist-Memphis. + Ensures compliance with state, federal, and organizational guidelines, as well as facilitates fulfillment of registry guidelines. + Performs other related duties as assigned.
