**Responsibilities:**
AmeriHealth Caritas Family of Companies is growing and we are looking for a Corporate Population Health Program management with a focus on the population health continuum to join our team. ;If you are looking to challenge yourself, provide thoughtful leadership and improve managed healthcare industry this position is for you. ;We are looking for a work at home associate to help transform the healthcare industry aiming their focus on providing equitable health care.;
**Major Accountabilities**
As the Program Manager, you'll be accountable for assisting in the creation of Population Health Care Models in expansion and retention markets. You will concentrate on market priorities, identified gaps and enterprise strategies with the goal of enhancing outreach, engagement and ultimately boosting health outcomes, while concurrently decreasing overall expenses, driving business growth and retention.
+ In this role, you will collaborate closely with corporate and market clinical leadership, healthcare economics, capture planning, proposal teams and other cross-functional departments to communicate contractual mandates and establish a standardized corporate model. You will also account for innovative, market-specific requirements and differentiators.;
+ Conduct a thorough analysis of gaps in the Request for Proposal (RFP) and present the findings to key leaders regarding the identified gaps and potential solutions.
+ Incorporate an assessment of corporate and market clinical operations, interventions, and value-added services for population health management, while recognizing contractual obligations and market distinctions to demonstrate the effectiveness of clinical programs, interventions, and solutions.
+ Evaluate and enhance the member journey to develop an optimized, efficient, and impactful experience that enhances overall health and clearly highlights the experience of our AmeriHealth Caritas members, demonstrating the value of contracting with us to State entities.
+ Collaborate with Population Health Leaders to document, evaluate requirements, and create key solutions in preparation for Request for Proposal responses.
+ Maintain and facilitate the creation of key vital partnerships between corporate population health and our internal cross-functional teams, including network, member engagement, behavioral health, vendor management, utilization management (UM), care management (CM) and analytics.
+ Redefine and expand existing care coordination programs and interventions as needed, to enhance their value to current partners and promote further market growth and expansion.
+ Present at meetings with new business stakeholders to encourage effective communication and implementation of goals for both new business acquisition and retention programs. These meetings aim to facilitate a productive exchange of ideas and strategies that will promote the achievement of program objectives.
+ Execute department-specific special projects, as assigned Director with Population Health, to support the departments functions and requirements.;
**Knowledge, Skills, and Abilities**
+ Knowledge of population health clinical interventions, including clinical frameworks, business models and key program objectives.
+ Prior experience with Managed Care Organizations (MCOs) to assist with responses to RFQC, RFI and RFP's.
+ The ability to analyze and track outcomes reporting results and assist in creating effective solutions.
+ Business writing and/or RFP experience.
+ Experience in Change Management.
+ Strong interpersonal skills demonstrated by compassionate, courteous, cordial, cooperative and professional interactions with diverse groups of co-workers, community partners, clients, and members of the community.
+ A clear understanding of AmeriHealth Caritas's mission, goals and objectives, and the ability to work independently with high energy and contribute as part of a larger team.
+ The ability to efficiently organize tasks and pay strong attention to detail in follow-up and follow through.
**Experience, Education, and Licensure**
+ Bachelor’s Degree in Healthcare, Public Health or related field required.
+ Current and unencumbered Registered Nurse or Licensed Clinical Social Worker required.;
+ 3+ years of experience in healthcare program management;
+ 5+ years of progressive healthcare experience with a specific focus on women's health and maternity.
+ Experience in healthcare program management, program oversight, digital healthcare solutions or healthcare vendor management (within a managed care organization).
