As a Crisis Facilitator, you will provide critical crisis intervention and support services to youth and families facing severe psychological, emotional, and behavioral challenges. This position involves providing services in diverse environments, including client homes, hospitals, and community spaces. The Facilitator is responsible for coordinating and implementing treatment goals and service delivery for assigned clients, engaging regularly with both clients and caregivers.

 

ABOUT IMMEDIATE STABILIZATION SERVICES (ISS)

The ISS program provides prevention and stabilization-oriented services designed to support young people and their Resource Families in times of crisis and high need to reduce the likelihood of placement disruption and preserve their placement. ISS staff are available up to 24 hours a day to provide phone-based assessment, in-person stabilization, and short-term intensive services designed to improve youth safety and well-being. ISS services are also utilized to facilitate successful transition into new placements for youth as they leave the Welcoming Center. In this way, the ISS team can be a stabilizing and consistent support team for young people as they experience a change in placement.

Responsibilities Facilitate Child and Family Planning Meetings, including preparation, coordination, facilitation, note-taking, and follow-upManage cases and coordinate care with county and community partners (e.g., social workers, therapists, probation officers, teachers)Complete all necessary documentation, including mental health notes, Family Team Meeting notes, and incident reports in a timely and accurate mannerProvide crisis management and intervention, including behavior management if necessaryAttend team meetings, trainings, and supervision sessionsFollow clinical best practices and complete required clinical documentationParticipate in emergency on-call response once every quarterUtilize crisis communication and de-escalation techniques as needed Qualifications

REQUIRED

Bachelor’s degree2 years’ experience working with children/youth and families with complex and enduring needs in a mental health settingFlexible schedule with the ability to work some evenings and weekend hours as neededParticipate in emergency on-call response once every quarterMust be at least 21 years of ageOwn vehicle. Valid driver's license, clean driving record, and insurability through Seneca Family of Agencies’ insurance policyTB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirementsProof of up-to-date COVID-19 vaccination
