JOB DESCRIPTION:

SUMMARY OF POSITION

Engages with individuals and/or families experiencing a recent crisis and assists with navigation of the mental health system and community resources. Travel across the statewide service area is required. Fosters relationships with existing external partners by participating in community meetings, committees, educational forums, etc. Actively works to establish new relationships with external resources within the community. May participate in logistical planning, overcoming obstacles, addressing stigma and providing support to clients. Performs a continuing review of the client’s needs and functioning, maintenance of appropriate documentation, links clients to appropriate services and advocates on their behalf. Provides support in an active treatment environment, client-centered consultation.

ESSENTIAL DUTIES & RESPONSIBILITIES

Fosters relationships with existing external partners by participating in community meetings, committees, educational forums, etc. Actively works to establish new relationships with external resources within the community. May participate in logistical planning, overcoming obstacles, addressing stigma and providing support to clients. Travel across the statewide service area will be required.Assists clients with making linkage with internal and external resources and maintains appropriate contact with referral sources including providing information on client progress, behavior, and needs (with appropriate releases), and maintains ongoing consultation when appropriate. Immediately begins the engagement process with newly assigned clients and their families by documenting contact within 2 business days of the referral.Provides intensive community-based services utilizing evidence-based methodologies to provide excellent clinical services, follows up with all clients on caseload in accordance with outreach protocols.Demonstrates commitment to all assigned clients and families at various stages of change by using motivational interviewing and outreaching clients who have disengaged from services.  Provides assertive outreach, advocacy and follow-up to assigned clients. Demonstrates effective caseload management by expeditiously staffing and transitioning client cases based on client needs and preferences.Completes and submits all required clinical documentation within established timeframes.  To ensure a complete clinical record, completes Progress Contact Notes for all contacts, canceled/failed appointments and attempted contacts with clients and collaterals within established timeframes.Attends staff meetings, clinical staffings, and other internal and external meetings as assigned.  Participates on committees as assigned, including quality assurance functions.  Maintains appropriate communication with other employees and works as an effective member of treatment team.

KNOWLEDGE, SKILLS & ABILITIES

Effectively communicate via written, verbal, in person and virtual methods.Knowledge of community geographical awareness and of community and agency resources.Skill in customer service techniques.Knowledge of appropriate billing codes.

QUALIFICATIONS

Education Level

Bachelor’s degree required.

Years of Experience

Experience navigating multiple youth and/or adult mental health service systems preferred.

Certification/Licensure

Must meet Mental Health Professional status.

CPR Certification preferred.

$17.35--$24.30
