Position: Spring 2025 Unpaid Fieldwork Placement for Neighborhood Care 1115 Waiver Implementation

Program description: EmblemHealth is a nonprofit, mission-based health plan headquartered in New York City serving 2 million New Yorkers. EmblemHealth Neighborhood Care provides free, in-person services and support, connections to community resources, and health and wellness programming to community members at 15 locations throughout New York City. New York State Department of Health’s new 1115 Medicaid Waiver aims to enhance delivery of social care services for Medicaid members through the creation of Social Care Networks (SCNs) that will coordinate community-based organizations (CBOs), providers, health plans, and other health partners to improve services for Medicaid members. This role would support Neighborhood Care’s participation in the waiver as a CBO partner.

Fieldwork Position Overview: 
This role will support the AVP of EmblemHealth Neighborhood Care and help ensure that the team meets greater program goals and deliverables in a timely and effective manner. The position’s responsibilities will include working closely with Neighborhood Care leadership and staff to help develop and implement new processes for working with each regional SCN and screening and referring Medicaid members for social care needs. 

This is a fieldwork position, totaling 180 hours from January – June 2025. (~10 hours/week) The mode of work for this position will be remote / hybrid. The position will conclude at the close of the CUNY SPH spring semester.  

Neighborhood Care 1115 Waiver Implementation Intern Fieldwork Responsibilities: 
•    Develop knowledge and understanding of each regional SCN’s program requirements
•    Work with Neighborhood Care managers and staff to access SCN IT platforms and organize training resources to support implementation
•    Work with Neighborhood Care managers and staff to develop and implement workflows for screening and referral 
•    Help to develop project plans and trackers for establishing new processes related to waiver service implementation 

Qualifications: 
•    Student at CUNY SPH in good academic standing
•    Excellent organizational and project management skills
•    Ability to gather and synthesize information and report to a variety of stakeholders
