Job Overview: We are seeking a highly skilled and compassionate Nurse Practitioner to join our dynamic team as a Long-Term Care Nurse Practitioner. This position offers a unique opportunity to contribute to the well-being of our community's aging population by providing comprehensive healthcare services in long-term care settings. The ideal candidate will be an experienced and caring professional committed to delivering patient-centered care with a focus on quality of life. Responsibilities: * Conduct comprehensive assessments of patients in long-term care facilities, including medical history, physical examinations, and medication reviews. * Develop and implement individualized care plans to address the unique needs of long-term care residents. * Collaborate with interdisciplinary healthcare teams to coordinate and optimize patient care. * Provide education and support to patients, families, and caregivers on disease management, preventive care, and overall health and wellness. * Monitor and manage chronic conditions, collaborating with physicians and specialists as needed. * Participate in regular team meetings and contribute to the development and improvement of patient care processes. * Stay abreast of current healthcare trends, regulations, and evidence-based practices related to long-term care. Qualifications: Master's or Doctorate degree in Nursing from an accredited program. Current and unrestricted Nurse Practitioner license in the state of Maine. Certification as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner. Minimum of 2 years of experience as a Nurse Practitioner, preferably in long-term care settings. Strong interpersonal and communication skills with the ability to collaborate effectively with diverse healthcare professionals. Dedication to providing compassionate and patient-centered care. Knowledge of electronic health records(Epic) and proficiency in relevant medical software.
