OverviewThe Dementia Program Activities Coordinator will work with dementia patients receiving hospice care at home and in facilities to find activities that engage the interest of persons with Alzheimer's disease and other dementias. The Coordinator models activities to family caregivers and facility staff. The Coordinator will also perform bathing consultations in which they will model strategies to maximize the comfort of both the person with dementia and their caregiver. Additionally, the Coordinator will function as an IDT member for the patient, providing information to other care team members during IDT and individually, as appropriate.
Responsibilities Speaks with family members/caregiver, facility staff and Haven team to identify:Areas of interest and engagement opportunities for patient Barriers to bathing and/or other ADLsDemonstrates advanced knowledge of providing engagement and care to patients with dementia, as well as available community resources for patients and caregiversDemonstrates proficiency in active listening skills and exhibits a non-judgmental, empathetic professional personaSchedules on-site visit with patient and caregiver and/or facility staffCreates an assortment of individualized activities to attempt with patient including music, textiles, dolls, stuffed animals, picture books, etcVisits patients in private homes and facilities to model engagement activitiesAdjusts actions based upon the situation and the response of the patient and family/caregiverCreates a customized activity guide to share with family, caregivers, and the Haven teamCreates an individualized plan for visit utilizing materials such as bath ponchos, rinse less shampoo, music, etcVisits patients in private homes and facilities to model bathing techniquesAdjusts actions based upon the situation and the response of the patient and family/caregiverCreates a customized bathing guide to share with family, caregiver, and the Haven teamCompletes documentation of visit in EMRAdds customized activity and ADL strategy guides to EMR and shares during IDT and/or individually with care team membersBi-monthly follow-ups with the Haven team to assess how the engagement and bathing/ADL strategies have been implemented and if needed, offer additional suggestions QualificationsEDUCATION/EXPERIENCE
• High School diploma or GED equivalent, preferred.
• One (1) to two (2) years CNA experience preferred.
• Experience working with interdisciplinary team preferred.
LICENSE/CERTIFICATION/OTHER SPECIAL REQUIREMENTS
• Current Certified Nursing Assistant Certificate required.
• Driver's license and insurance required.

• Ability to work non-judgmentally with persons of varied cultural, religious and socioeconomic backgrounds and lifestyles.
TRAVEL REQUIREMENTS
Percentage of Travel: 25-75%
**To perform the duties of the position will require frequently sitting, standing, and walking, and occasionally bending, reaching, climbing (stairs/ladders), crawling, kneeling, crouching, stooping and typing on a keyboard with fingers. The phyiscal requirements will be the ability to push/pull and lift/carry 21-30 lbs**
