Job Summary: The main focus of the DDP is to coordinate the development and implementation of supported living services with referral source, consumer and employee, ensuring the delivery of quality services. This coordination occurs through the direct supervision of employees, supports for consumers, and ongoing communication with family, community, funding and licensure sources.

The DDP will educate and support clients and caregivers throughout our Savannah, GA territory.
 

Essential Duties & Responsibilities:

Actively participates in the person centered planning process, including development and implementation of Person Centered Plans (PCPs) and the Individual Support Plan (ISPs)Design and implement support and teaching strategies targeted at assisting consumers with reaching their outcomes as outlined in the consumer’s ISPProvide supervision and training to Direct Support Professionals regarding the implementation of the PCP, ISP and teaching strategiesEvaluate and revise support and teaching strategies as necessary to ensure progress toward achievement of outcomes and action stepsRegularly monitor the implementation of the PCP and ISPRevise PCP and ISP outcomes as necessary based on data collection, feedback from staff, and direct observation of the consumerEnsure that services and supports are provided in the PCP and ISPFacilitate opportunities for natural supports & document specific QDDP activities provided to the consumerCoordination of consumer benefits including Medicaid, Medicare, SSI, SSA, and Food StampsSecuring community resources for consumers supportedScheduling and completion of consumer medical needs

Quality Assurance Related Functions:

Review all forms, notify Case Manager of any incidents, and provide oversight for the follow up plan with staffMaintain communication with family members, guardians, staff and Case Manager on a regular basis, particularly regarding policies, issues, and concernsPrepare and submit reports that are required by contract, funding sources, or regulations

Supervision/Training Related Functions:

Coordinate in-service trainings needed to assist staff in fulfilling responsibilities related to consumer directed supportProvide oversight with documentation of the Medication Administration Record with checks of the MAR at least once per weekMonitor and record required trainings for staff and schedule updates as needed to remain current. Complete monthly staff schedule ensuring appropriate levels of staff coverageProvide direct support in providing services needed to protect the health and safety of the consumer, in a staffing crisisProvide training and oversight for staff regarding Medicaid Waiver guidelines and licensing and certification standardsCommunicate with the Interdisciplinary Team about concerns and findingsBe available to respond to staff in case of an emergencyPerform on-site evaluations for all field staffComplete all annual evaluations and submit to Manager one month prior to hire date anniversary to ensure timely reviews

Education and Experience:

BA/BS Degree in healthcare related field requiredExperience in the field of developmental disabilities (preferred, not required)Supervisory experience is a plus
