The Center for Health Promotion is seeking a Diabetes and Behavioral Health Unit Supervisor to join their team! This position is responsible for leading statewide efforts to reduce the impact of diabetes and arthritis in Minnesota. Serving as a technical expert, this position oversees the development, implementation, and evaluation of statewide initiatives aimed at improving health systems, fostering community-clinical linkages, driving policy changes, and advancing community-based education and public awareness. A core focus is reducing health disparities through research-driven strategies and evidence-based practices. In addition, this position will oversee the implementation and evaluation of strategies to increase participation in evidence-based programming, build and create new partnerships to advance work of disease prevention and management, and expand use of effectives approaches that are found to be effective and sustainable. Key responsibilities include: + Identify diabetes and arthritis emerging needs and issues in Minnesota and guide statewide planning and intervention efforts. + Build capacity and direct planning, implementation, and evaluationof statewide programs for health systems improvement, community clinical linkage development, policy change, data and surveillance, community- based education, and public awareness. + Maintain a high degree of scientific knowledge about diabetes and chronic disease epidemiology, prevention, treatment and management, and how behavioral and social risk factors impact onset and outcomes and health disparities. + Supervise work of the Diabetes Unit staff and assure that they receive training and support needed to successfully implement the goals and objectives of the Unit’s activities. + Oversee and coordinate preparation of grant proposals and renewal applications for research studies and diabetes prevention and control initiatives. This position is eligible for telework to applicants who reside in Minnesota, Wisconsin, Iowa, North Dakota or South Dakota. Position locations will be determined by the top candidate and will be based out of one of the following MDH district office locations: **St. Paul, St. Cloud, Duluth, Rochester, Mankato, Marshall, Bemidji, Fergus Falls.** Reimbursement for authorized business travel expenses is provided according to bargaining unit guidelines. **Minimum Qualifications** _To allow for proper crediting during the review process, please ensure your experiences clearly communicate the established minimum qualifications._ Four (4) years of experience in public health program planning, implementation, and evaluation that include at least three (3) of the following responsibilities: + Experience implementing statewide initiatives focused on chronic disease prevention and management + Experience building and maintaining partnerships with diverse stakeholders, including tribal nations, community organizations, health systems, and governmental entities. + Experience managing grants and budgets, including federal and state funding sources. + Experience as a supervisor, team lead or project lead OR completion of a State of Minnesota leadership program (e.g. ELI, L4, Everyday Leaders) Qualifications to be assessed during the interview phase: + Strong written and verbal communication skills. + Ability to convey technical information to non-technical audiences. + Familiarity with health systems improvement, community-clinical linkages, and policy advocacy strategies. A related master’s degree may substitute for eighteen (18) months of experience; bachelor’s degree may substitute for twelve (12) months of experience; and associate’s degree may substitute for six (6) months of experience. **Preferred Qualifications** + Experience implementing diabetes, arthritis and chronic disease prevention and management programs. + Experience promoting evidence-based lifestyle change programs such as the Diabetes Prevention Program. + Experience building and maintaining community partnerships **Additional Requirements** This position requires successful completion of a background check and employment reference check.
