Responsibilities Develop diabetes self-management education and pre-diabetes related education resources for patients referred for consultations. This includes patient assessment and education on diabetes self-management Provides education and training for care managers, medical assistants and nursing staff regarding diabetes self-management using the 3 M of self-care and other care plans/resources in alignment with ADA guidelines. Assess clients’ nutritional, health needs and social determinant needs Conduct in person, Televideo and groups education and counseling Participates in community events on the topic of healthy eating and diabetes education. Documentation care plans in Epic Evaluate if the meal plans have a positive effect on clients’ health Create educational material about healthy eating habits Speak to groups about the importance of nutrition to prevent specific diseases Document patient progress Qualifications Job Requirements: Certification as a Diabetes Educator Registered dietitian in the state of Oklahoma OR Registered nurse in the state of Oklahoma Preferred Job Requirements: Three to Five years of experience as a dietitian or RN
