Diabetes Educator - Diabetes Self Care Center

The Diabetes Educator aims to help improve the overall quality of life for patients living with diabetes and their families. Under the direction of the Coordinator, Patient Education, you will provide diabetic patient education for individuals and groups while collaborating with other disciplines in the Diabetes Self Care Center.  

A caregiver in this position works from 8:00AM – 4:30PM. 

A caregiver who excels in this role will:  

Assess the learning needs of the patient with diabetes. Plans educational goals to meet identified learning needs of the patient, communicates with patients and families, and documents plans of care. Teach diabetes education classes and develop and revise education curriculum as appropriate. Assist in formulating policies, procedures and standards of practice. Coordinate bi-monthly Diabetes Education Advisory Committee meetings and participate in hospital quality improvement activities. Monitors fiscal plan (budget) operating within parameters. Evaluate and set standards for facilities and equipment and monitors utilization. Promote good public relations for the Center and Hospital. Keep accurate records and send appropriate forms to physician's office. Monitor productivity by keeping monthly outpatient / inpatient statistics. Evaluate the effectiveness of diabetes patient education program through the designated follow-up program. Collaborate with other healthcare team members to coordinate medical and nursing management of patient care. Utilize research findings in clinical practice and / or participate in research activities. Demonstrate a proactive approach in patient education and discharge planning for patients with diabetes. Assist in orientation of RN's and other hospital personnel and serve as a resource to nursing, physicians and other departments. Attend meetings and seminars to enrich personal knowledge, growth and skill in providing care for patients with diabetes. Serve on internal and external committees in support of diabetes education. Complete mandatory education and training to maintain organization and department specific competencies and requirements and applicable certification/licensure 

Minimum qualifications for the ideal future caregiver include:  

Graduate from an accredited school of Nursing Licensed and currently registered as a professional nurse in the State of Ohio Certified Diabetes Educator OR successful completion of National ADA exam after 1000 hours Minimum of two years recent clinical experience with emphasis on the care of diabetic patients 

Preferred qualifications for the ideal future caregiver include:  

Bachelor's Degree in Nursing, Master's Degree in Nursing Education or related field
