Diabetes Educator - Maternal Fetal Medicine Location: WellSpan Medical Group, York, PA Schedule: Full Time Schedule Full Time: Monday-Friday 8:00 am - 4:30 pm General Summary Provides group and individual Diabetes Education. Provides nutritional counseling to clients referred by a provider. Develops educational goals, learning objectives and a plan for educational content and teaching methods. Develops an individualized follow-up plan with each program participant. Duties and Responsibilities Essential Functions: + Provides group education and individual education in Diabetes Education. + Provides nutritional counseling to clients referred by a provider. + Collaboratively develops educational goals, learning objectives and a plan for educational content and teaching methods with program participants. + Manages blood glucose for patients. + Manages insulin pumps and continuous glucose monitors for patients. + Collects program participant assessment data, using the ADDE7 framework, in a collaborative and ongoing manner. + Collaboratively develops an individualized follow-up plan with each program participant. + Conducts a follow-up assessment upon completion of program services, using outcome measures from the AADE7 Continuum of Outcomes Framework. + Documents assessment data, educational plan, educational services provided and evaluation results in each participant's educational records. + Utilizes a team approach to provide services and collaborates and communicates with team members when needed. + Participates in peer review process to evaluate performance of other professional staff.
