Responsible for providing ongoing education and supporting individuals with diabetes throughout their lifespan. Provides education in group and individual settings for patients with Type 1, Type 2, Gestational Diabetes, Pre-Diabetes and Metabolic Syndrome. Provides education tailored to special learning needs.


Principal Accountabilities:

Provides education in group and individual settings for patients with Type 1, Type 2, Gestational Diabetes, Pre-Diabetes. Provides education tailored to special learning needs.

Coordinates individualized patient and support system assessment, education, planning and implementation of care goals.

Collaborates with the referring providers and interdisciplinary team to work toward optimal patient education outcomes and glycemic control. 

Maintains professional education standards required to be a CDE/CDCES in an ADA approved Diabetes Self- Management Education program.
