Responsible for the coordination and oversight of outpatient diabetes and nutritional education services. Works collaboratively with the Medical Director(s), physicians, inpatient diabetes and nutrition resources, allied health providers, physician organizations and the community. Develops, implements and evaluates patient and professional education. In collaboration with St. Mary’s Health Care System leadership, is responsible for establishing a plan for continuous improvement in program quality, program volume and community outreach. Provides supervision and coordination to the interdisciplinary team that includes registered dietitians, registered nurses, program instructors, student interns, clerical support and volunteers. The program coordinator functions as a program liaison across the continuum and is responsible for program outcome measurement as well as certification of the diabetes education program.
Position Requirements:
A. Education: Registered Dietitian. Master’s degree preferred in Nutrition. Certification in Diabetes Education through the National Certification Board for Diabetes Educators must be obtained within six months of hire.
B. Experience: Registration in field required. Prefer 3-5 years in clinical, outpatient, diabetes and or related field with management experience.
C. Special Qualifications: Must have strong interpersonal skills to communicate effectively with a diverse patient population, peers, staff and physicians. Analytical ability to evaluate clinical and operational data for program monitoring and evaluation.
Leadership skills and ability to coordinate a clinical team and fosters a positive and engaged work team. Writing skills sufficient to prepare written material in a clear, concise, and organized manner.
Good organizational skills with ability to prioritize multitude of responsibilities and duties.
Ability to type on a computer keyboard and good working knowledge of computer software programs. Advanced teaching and presentation skills.

Identifies and recommends program goals and objectives for the St. Mary’s Nutrition & Diabetes Education services and facilitates staff participation to reach the desired outcomes. Coordinates daily operations by working with Patient Access; overseeing scheduling of clinic staff, activities of clerical staff and volunteers, scheduling, registration, billing of patients, and documentation and implementation of services.Seeks out opportunities to expand and enhance diabetes and nutrition services relative to the demands of the patients, physicians, and the community.Develops, implements, monitors, and evaluates standards, policies and procedures related to the delivery of diabetes education and nutrition services in alignment with Joint Commission requirements.Takes the lead for maintaining updated diabetes curriculum, educational materials, and programming to provide current content based on changes in research and the health care industry.Prepares and organizes written reports to credentialing agencies and for community benefit accounting as applicable in a timely manner. Conducts data analysis and maintains statistics as applicable for certification and program review.Manages and updates diabetes and nutrition databases as needed to collect outcome data required for program certification(s).Coordinates educational programming, in-services for SHLA staff, as well as other community providers. Establishes work priorities, assigns work to and monitors clinical performance of staff.Participates in the training, orienting and scheduling of the clinical team.Facilitates the selection and monitoring of key indicators of performance (measures of success) relating to patient satisfaction, patient access/participant volumes and clinical outcomes.Provides input into the preparation and management of annual departmental budgets. Estimates future needs and makes recommendations. Communicates pertinent information, statistics, problems and plans so that there is an excellent understanding of the programs, activities, issues, and opportunities.Recommends strategies to address regulatory changes and improve patient care in a timely, cost-effective, and safe manner.Takes a leadership role in implementing changes in programs, practices, procedures and/or systems within the clinical and operational work area.Maintains current clinical practice skills including Registered Dietitian and Certified Diabetes Educator status to maintain in-depth understanding of clinical nutrition services.Identifies and plans to meet continuing education requirements in education through participation in seminars, workshops, and in-services.Serves as a representative for the work group, departmental representative/liaison and develops relationships with internal groups as well as external groups such as physician groups, community groups, Georgia Department of Community Health, other diabetes education programs and state and national diabetes, nutrition, and health organizations.Maintains current clinical practice skills by providing direct care in both individual and group settings.Monitors productivity and resource utilization. Maintains records of departmental productivity.
