Diabetic Coordinator

Major Responsibilities:

Case management of sub-specialty, pediatric patient populations for coordination of care.  Plans, monitors, and coordinates individual patient care plan.Utilize critical pathways and best practice guidelines as appropriate for both inpatient and outpatient needs.   Collaborates with the patient, family, specialist, pediatrician, and multi-disciplinary care team to assure appropriate, safe, cost effective, timely, and efficient care.    Organizes, educates, and schedules (if needed) pediatric, sub-specialty procedures, appointments, nurse visits etc.  Coordinates patient care conferences and educational offeringsActs as the primary point of contact for patients and their families assisting with all compacity of care, triage concerns, medication education, medication refills, and all other requests within scope.Participates in continuous quality improvement processes; assists in determining issues to be examined participates in and coordinates data collection and analysis.   Documents in patient’s medical record as appropriate.Assists in initiating and implementing necessary changes in patient care delivery through the sharing of research and current trends.  

Job Requirements

Education

Bachelor of Science in Nursing (BSN)

Certification / License

Certification in your current area of specialty within 2 years or 2,000 hours, if applicable

Work Experience

Ideally, 1-2 yrs pediatric nursing experience of that specialty, otherwise 1-2 years nursing experience

Knowledge / Skills / Abilities

Basic office skills (computer/typing)Strong clinical/ assessment skills.Strong assertive skills - Communication skillsAbility to cooperate and work well with othersMay be required to cover weekends, holidays and off shiftsExcellent interpersonal skillsCPR certificationCurrent license in the state of Illinois as a nurse.

Physical Requirements and Working Conditions

Ability to demonstrate a commitment to patient centered care.Ability to work under stressful conditions and in difficult situations.May exposed to hazardous materials and life threatening diseases.Ability to perform heavy lifting/transfer activities related to patients.Ability to work rotating shifts any day of the week.Ability to transport objects from one location to another.Ability travel to, and work in outreach clinics.This position will need to recognize needs and behaviors of a variety of age groups of patients treated (i.e. understand growth and development.If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
