Essential Functions
Nutritional care of patients. Assist in maintaining the diabetic and nutrition education program that meets ADA certification. Assist in the diabetes education of hospitalized and outpatient diabetes patients. Assess, evaluate, educate and help patients formulate an education plan based on the national standards for diabetes self-management education. Help patients identify goals and objectives based on a core body of knowledge in diabetes content topics and self care behaviors. Consult with all necessary disciplines to ensure instructional and educational needs of patients are being met. Accurately documents patients’ treatments and education requirements. Assists in the development of new programs for patient education. Serves as clinical resource for nursing staff, physicians, therapists, and other medical personnel in diabetes and nutrition care. Assist with educational activities, in-services, skill fairs or other activities to provide education to staff for the maintenance of competence levels in diabetes & nutrition care. Maintains statistical data for outcome and benchmarking studies to continue ADA Personal Standards Maintain professional growth and development through seminars, workshops and professional affiliations to keep abreast of latest trends. Meet the education requirement to maintain program’s recognition and certifications. Coordinates and promotes a positive relationship between staff, physicians, patients and families. Provides leadership and assistance in marketing the services of the diabetic and nutrition education department. Nutritional care of patients. Conducts diet consultations with patients, families, and others of the medical team in in-patient and out-patient setting. Works with inter-disciplinary team to assess the need for nutrition intervention and education for all patients. Charts nutrition assessment and plan of care given in patient’s medical record, whether orally or parenterally. Maintains all required records, reports, and statistics. Chart notes according to hospital policy. Updates nutrition education materials as needed, provides age appropriate materials-children, adolescent, adult, and geriatric. Aware of and knowledgeable regarding the developmental needs of patients across the age span. Assists Director with diet manual review annually and updates as needed. Available as education resource to nursing and medical personnel, patients and their families. Represents Nutrition Services in all clinical functions. Assists Director with Nutrition Services policy and procedures manual. Patient Menus. Develops or revises modified diets as needed. Writes menus for diabetic, low sodium, and other modified diets as needed, individualizing to the patients’ needs as requested. Visits patients to determine dislikes, allergies, requests, special nutritional problems or inadequacies and nutritional assessment. Charts any significant findings in the patient’s medical record, and modifies the patient’s diet according to the physician’s orders. Patient menus are checked for nutritional adequacy with each revision of cycle menus. Menus will exceed 100% of the USRDAs for protein, vitamin C and D. Personal Standards. Uses body language to convey a sense of caring and warmth. Maintains eye contact with patient. Uses active listening skills. Maintains RD status; keeps registration current, pays dues annually. Attends special in-services and educational seminars as requested by Assistant Administrator. Shows willingness to learn and accept change when presented by Assistant Administrator. Arranges for dietitian coverage when sick leave/vacation. Work with community services and other organizations that are associated with the Nutrition Department.Qualifications and requirements
