The Vascular Access Coordinator is responsible for the coordination of care and management of patients’ vascular accesses and the communication and collaboration with the Vascular Access Team.

 

Schedule: Potential fulltime shifts vary from four 10-hour shifts or three 12-hour shifts and are set upon hiring; Sundays off; no overnight shifts

 

Compensation: Pay range from $25 per hour, depending on nursing and dialysis experience

What You Can Expect:

Focus on vascular access care and management to increase permanent access longevity (AVF or AVG)Maintain vascular access data to assure tracking and trending of dialysis therapy and outcomes monitoring for CQI / QAPI activitiesProvide leadership and communication among caregiversEducate staff and hemodialysis patients in the care and management of vascular accesses, with particular attention to opportunities for improvement.Monitor vascular access and hemodialysis care delivery with particular focus on identifying access dysfunction and opportunities to increase arteriovenous fistulas (AVFs) and reduce catheters.For accesses with dysfunction or other pathology, assist with referral for diagnosis or surgical treatment Participate in Network 7 CQI activities to improve patient and facility vascular access outcomes.Complete other vascular access-related tasks as directed by Nurse Manager.  Qualifications

Education/Training:

Six months experience as hemodialysis professionalCertification as hemodialysis technician or licensure as LPN or RNDemonstrated proficiency in vascular access care and managementDemonstrated commitment to continuous quality improvement (CQI)Excellent communication, writing, and organization skillsBasic computer skills
