Learn more at HonorHealth.com. Responsibilities Job Summary Under the direction of the registered dietitians, provides nutrition care services to hospitalized patients. Participates in integrated nutrition screening processes, assessment of nutrition status, and care plan development and implementation. Documents patient care in the medical record. Monitors and evaluates nutrition status; develops meal and nourishment plans to meet nutritional requirements. Provides survival skill, discharge nutrition education to patients and families. Refers high risk patients to the registered dietitians. Generates nutrition screening reports. Following established guidelines, rates individual patient needs and nutritional status relevant to patient age and condition. Refers high risk patients to the registered dietitian. According to Nutrition care standards, develops and implements a plan of nutrition care incorporating available data. Documents nutrition care in patient medical record in a timely and accurate manner. Evaluates effectiveness of nutrition care plan. Monitors progress of patients’ achievement of nutritional goals. Reassessment of nutrition status is completed in accordance with nutrition care guidelines. Assesses education needs and provides individualized education to patients and families. Provides basic nutrition education, i.e. survival skill teaching, to patients and families prior to hospital discharge. Initiates calorie counts and documents nutrition analysis in the patient medical record. Sets up and processes between meal nourishments for patients desiring or requiring them. Verifies tolerances and changes supplements accordingly. Attends multi-disciplinary patient care conferences and actively participates, promoting discussion of patients current nutritional status and suggests nutrition care plan. Qualifications Education Bachelor's Degree in Food and Nutrition or Associates Degree in Dietetics with ADA Registration as Dietetic Technician Required Experience 6 months in healthcare setting Required Licenses and Certifications
