Summary In accordance with the National Scope and Standards of Diabetes Educators, as well as the National Standards for Diabetes Education and Support, the registered dietitian nutritionist assesses the needs of patients and designs, implements and evaluates a teaching plan. The registered dietitian nutritionist provides individual nutritional counseling in the following areas (to include, but not limited to): diabetes, prediabetes, obesity and weight management, hyperlipidemia, cardiac risk management, hypoglycemia, general nutrition and wellness, pregnancy, gestational diabetes, G.I. related disease problems, liver disorders, underweight and malnutrition, cancer, kidney disorders,and pulmonary nutrition. The Dietitian is also responsible for facilitating and assisting in the development of programs for group sessions, seminars, cooking demonstrations, supermarket tours, and lectures for the local businesses/community. Interacts with an interdisciplinary team of health professionals. Promotes positive behavior change with client population using motivational interviewing techniques, goal setting and effective client communication and support. Evaluates and assists clients with barriers to change. Essential Duties and Responsibilities 1. Provide one-on-one and group outpatient medical nutritional counseling to include, but not limited to: Healthy Me-Diabetes, Diabetes Prevention Program, Why Weight, My Healthy Weigh, and Chronic Disease Management. 2. Conduct group sessions relative to weight loss and maintenance, diabetes management, cardiac risk management, pulmonary management, and general nutritional wellness 3. Provide diabetes self-management education and support in all 9 content areas as defined by the National Standards of Diabetes Self-Management Education and Support 4. Assess the nutritional needs of outpatients and develop nutritional care plans with prescribed therapeutic diets for nutritional problems 5. Educate patients about behavior modification relative to diet 6. Document all information on the progress of each individual in patient electronic records 7. Communicate with patient’s provider or provider’s office to report on patient’s progress 8. Assist in the implementation of the assessment plan, and in the implementation and evaluation stages of the nutrition care process 9. Assist in developing and revising programs for individual and group interventions 10. Participate in the Diabetes Program Quality Improvement Plan 11. Participate on the Diabetes Advisory Committee 12. Facilitate programs, seminars, cooking demonstrations, supermarket tours, and lectures for the local businesses, the community, and the Lifestyle Center 13. Meet with local employers to determine employee needs and nutritional services being offered 14. Assist in answering phones to provide callers with program information 15. Provide information to patient program participants and the general public regarding Lifestyle Center programs and related services 16. Make appropriate referrals to Lifestyle Center and CRH programs 17. Perform other duties as assigned Education and Experience Minimum Required Education: Successful completion of a Registered Dietitian Nutritionist program Preferred Education: Bachelor or Master of Science Degree in Nutrition, Dietetics Experience: 2+ years in teaching nutritional concepts in a hospital or outpatient nutritional program Certificates, Licenses, Registrations: Applicants must be a Registered Dietitian Nutritionist through the Commission on Dietetic Registration (CDR) with current CDR status.
