Responsibilities

Comprehensive nutritional assessment of patients prioritized to be at risk during the screening process or as a requested consult.Develop appropriate interdisciplinary care plans based on current treatment knowledge and clinical practices, discussing care plan with patient and/or significant other and the appropriate staff.Respond to requests for nutrition education or counseling for patients and/or groups including appropriate materials for continuation of medical nutrition therapy or wellness.
