As a SAGE - Direct Support Professional, you will:

Provides 1:1 supervision, coaching, modeling, engagement, and support of students with intense needs in living, activity, school and community settings. Instructs, organizes, and assists with carrying out daily living activities, recreational activities, developing self-care skills (health/hygiene), learning and practicing social and peer interaction, community interaction and coping skills, budgeting for small personal expenditures, and schoolwork and/or homework.Supports and implements agency and individual treatment approaches for students as identified in individual treatment plan and/or education plans as established by a Licensed Therapist and the treatment team. These plans are specifically targeted for stabilization when admitted to GLS and transition into a group area.Works as part of an interdisciplinary team that includes therapists, case coordinators, SLPs, OTs, medical staff, education, program staff and families, collaborating to champion use of intensive behavioral and therapeutic interventions and strategies across programs with GLS.Under the direction of the Licensed Therapist employs innovation and creativity within daily treatment and therapeutic approaches.Collects specific data on target behaviors and target programming (as defined by the Therapeutic Services Coordinator) as well as, prepares and maintains written case notes after all shifts. Completes progress notes related to the implementation of interventions.  Monitors student progress through charts and graphs.Works with all internal teams when it is time to begin transitioning student into a specific group area by way of attending appropriate communication and/or team meetings to give feedback, discuss approaches, etc.Provides training, modeling, and consultation for program teams as students transition and integrate into standardized programming.During school assists students as necessary to meet classroom expectations, activity schedule, and behavior management program. Communicates with teacher, substitute teacher, and therapist about student progress, effective approaches, and concerns whenever necessary.Informs appropriate staff of students’ activities, behaviors and medical concerns. Completes and utilizes all required forms as outlined in policies and procedures, i.e. incident reports, communication log, bed checks, data sheets, shift treatment summaries, staffing reports, activity requests, activity logs, and other miscellaneous reports.Coordinates with team members to plan and implement on- and off-ground activity schedules and events for students. Organizes, time, space and people to promote the activity schedule.  

QualificationsMust be least 21 years of ageHigh School Diploma or GED required.  Associate or Bachelor's degree in psychology, education, or a related field preferred.1 year experience working with individuals with developmental disabilities in a therapeutic and supportive environment preferred.Valid WI driver's license with an acceptable driving record per company policy.Completion of additional training within 4 months of starting position.

 Schedule & Benefit Information
