Director, Case Management

POSITION SUMMARY The Director, Case Management for the Ardent Resource Team has responsibility for input and leadership for planning, managing, directing and evaluating activities and functions of case management departments across Ardent. The Director, Case Management is responsible for working in collaboration with the Ardent Case Management team for developing and implementing standardized workflows, tools, roles and systems to support case management services which includes case management and social work functions. The Director, Case Management assists the Ardent Case Management leadership team to establish metrics for achieving company targets/goals, care coordination services, regulatory compliance, and training new case management leaders. The Director, Case Management will perform duties and responsibilities in such a manner as to promote collaborative and professional behavior with case management team members, hospital leadership, medical and advanced practice provider staff, nursing staff, ancillary leaders, and other hospital team members. Responsibilities Assists the Ardent Case Management leadership team in formulating and implementing operational strategies and initiatives to meet the company’s standards for case management services. Intervenes with CM departments and campus officers to operationalize the activities and functions of CM services across the company to ensure quality, age/developmentally appropriate care is provided according the Ardent system mission, departmental objectives, policies and procedures, and internal/external regulations. Ensures a consistent effort toward quality improvement, employee engagement, patient centered care, and customer satisfaction. Works with the Ardent CM leadership team to lead operational improvements that facilitate standardization of case management services across the organization and across the continuum of care. Continually provides input for ongoing refinement of the strategic plan for case management services, identifies resources needed, understands risks and offers contingency planning. Responsible for input and strategic planning for the development, implementation, and education of action plans associated with identified areas of improvement in collaboration with the Ardent CM leadership team. Displays an ability to work effectively within the health system's decision making and organizational structures. Provides training and education to new CM leaders or identified employees in need of growth. Facilitates and participates in scheduled meetings with case management directors to enhance case management operations and identify areas of improvement. Works concurrently on a variety of tasks/projects to make timely and sound decisions in a fast-paced environment. Qualifications Education & Experience: BSN. Social Work degree, BSW, or related experience in case management. 3 years experience as Director Case Management experience in a hospital setting. 5 years acute hospital-based Utilization Review/Utilization Management experience, preferred.
