**Position Overview:**
Leads the organizational Pharmacy and Therapeutics, and Formulary Management process and lead the development of system-wide pharmacy programs and standards for clinical practice. Develops organizational approaches to integrate evidenced based formulary decisions into a common and standardized enterprise formulary across inpatient, outpatient, and medical Foundations. Provides clinical support and drug information to clinicians and coordinate the development of system-wide standards related to medication use policy. Coordinates the design and management of system-wide Medication Use Improvement initiatives that improve quality, affordability, and patient safety. Serves as the primary clinical pharmacy liaison to medical specialties regarding formulary, medication use standards, medication improvement initiatives, and evidenced based use of pharmaceuticals. Provides guidance, oversight, and sponsorship for system level medication committees and improvement initiatives.
**Job Description** :
**JOB ACCOUNTABILITIES:**
+ Lead the system Pharmacy and Therapeutics, and Evidenced Based Formulary Management process including clinical and financial analysis of medications and therapeutic classes, monograph reviews, clinical presentations, and monitoring and communicating the implementation of formulary decisions.
+ Develop and manage the annual system Pharmacy Optimization project plan. Develop evaluation and measurement plans to track progress and report results, engage with pharmacy directors, clinical leads, supply chain, and regional leadership to facilitate an enterprise plan and approach to medication use improvement.
+ Coordinate the development and implementation of system-wide medication use guidelines with local clinical pharmacy staff, pharmacy directors, electronic health record and subject matter expert physician leaders, nursing, and quality & safety leaders in affiliate hospitals and ambulatory settings.
+ Serve as primary liaison to medical and pharmacy clinicians regarding formulary, emerging new treatments and therapies, drug information, emerging pipeline and clinical therapies. Consult with organizational leaders regarding optimal strategies to improve medication affordability and manage pharmacy risk under a total cost of care environment.
+ Participate in the design & development of pharmacy strategic initiatives and outcomes evaluation methods to improve the clinical outcomes and affordability of medications. Consult in the development of practice innovation models for pharmacy and in the methods to evaluate their impact on the total cost of care.
**EDUCATION:**
+ Doctorate: Graduate of and accredited school of pharmacy.
**CERTIFICATION & LICENSURE:**
+ PHARMD-Licensed Pharmacist
**TYPICAL EXPERIENCE:**
+ 12 years recent relevant experience.
+ Leading formulary and utilization management for multi-site entities
+ Experience leading multi-disciplinary teams
+ Strong knowledge of FDA pipeline and industry trends
**SKILLS AND KNOWLEDGE:**
+ Extensive clinical knowledge of medical and pharmacy practice issues.
+ Basic knowledge of managed care strategies, drug benefit management strategies for pharmacy, hospital and medical group experience.
+ Knowledge of pharmaceutical outcomes measurement, cost/benefit analysis, value based clinical decision making.
+ Knowledge of student and resident teaching and mentoring tactics with the ability to work with pharmacy residents and interns.
+ Proficiency with MS Word, Excel, and PowerPoint and electronic health record systems.
+ Organizational, teamwork, interpersonal, time management, written and verbal communication skills.
+ Capability to set priorities amongst a series of competing clinical and operational priorities and to effectively direct the work of others.
+ Requires a basic working knowledge of legal requirements and accreditation standards including National Association of Boards of Pharmacy (NABP), The Joint Commission (TJC), Title XXII, United States Department of Homeland Security (DHS), Drug Enforcement Administration (DEA), Food and Drug Administration (FDA) and United States Pharmacopeia (USP).

Pay Range is $91.30 to $146.08 / hour
_The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs.
