Director, Home Care & Hospice- Full Time, Days, Morristown NJ

Reports to: Executive Director, Home Care and Hospice

 

Duties & Responsibilities: 

Develops, administers and monitors care delivery systems. Demonstrates knowledge of current policies and procedures, as well as, develops new policies and procedures, as needed. Demonstrates expert clinical knowledge by serving as a resource for the homecare team in areas of regulation, accreditation, professional standards, and the law. Develops processes across the continuum that delivers care in an efficient and safe manner. Ensures patient care models are consistent with organizational structure. Works with administration, subordinates, and Human Resources to ensure that staffing meet safe patient care standards. Demonstrates fiscal responsibility and accountability. Works collaboratively with administration to construct budgets. Establishes, monitors, and communicates budget performance and variance systems. Utilizes variance data to implement actions necessary to achieve budget compliance. Acts as a resource to staff in budgetary matters. Teaches strategies and empowers managers to operate within their budget. Demonstrates leadership with the Homecare and Hospice community. Oversees the management of the clinical service teams. Establishes and maintains open, consistent and regular communication with all levels of the organization. Establishes a leadership role in committees, task forces, and program development within Homecare and Hospice community. Identifies areas of potential risk and consults with the appropriate resources to eliminate or minimize those areas in a timely manner. Identifies strategies to increase competencies of team members.  Provides guidance and support in management decisions regarding clinical requirements and/or changes.

 

Plans and implements strategies to improve business and clinical efficiencies. Meet with management team to develop clinical expectations of staff. Meet with management team to develop clinical goals. Ensures appropriate resources are available for the delivery of services. Ensures professional practice model meets Atlantic Health mission, vision and values. Complies with New Jersey State licensing board requirements for professional staff. Participates in the planning, implementation and refinement of clinical programs. Demonstrates ability to track and trend profitability. Monitors aggregate case mix and utilization patterns. Monitors clinical outcomes. Ensures productivity standards are met. Monitors customer satisfaction data.  Critically analyzes the need for change and presents constructive, well formulated plans to rectify the identified deficit. Represents Management of Homecare and Hospice.  Participates in management meetings at site and system level. Participates in the survey preparation and process for Joint Commission and the Department of Health and Senior Services. Maintains leadership role in inquiries from compliance agencies for clinical competence and care delivery. Assures compliance with home health regulations as set forth by local, state, and federal law. Represents Homecare and Hospice in the absence of the Executive Director. Demonstrates knowledge and leadership ability in performance improvement activities. Participates in performance improvement initiatives. Identifies areas for development and implements changes as necessary to improve services and patient outcomes. Acts as a resource to managers in the formation, writing and monitoring of performance improvement plans. Utilizes data gathered as part of performance improvement in program development, problem resolution and budget formulation. Prepares and presents performance improvement data, as requested. Assumes responsibility for professional growth. Participates in professional organizations. Identifies educational opportunities which provide a growth opportunity in both clinical and administrative areas. Identifies areas of self-growth and formulates a plan to meet those needs. Maintains clinical knowledge in area of Homecare and Hospice. Meets standards of Atlantic Health leadership by creating, developing and achieving challenging goals that require business acumen, foresight, and resourcefulness.

Job Qualifications/Experience

New Jersey Nursing License; Master’s Degree in related field preferred, Bachelor’s Degree required Five years effective management experience in the healthcare field supervising small or large teams; 3-5 years progressive management experience in a homecare agency required; clinical experience in the areas of supervision; experience implementing change; experience with program planning; ability to identify areas of improvement; and ability to work with all levels of organization.
