Director of Ambulatory Quality and Population Health \n

Working closely with the Population Health, Quality, and Clinical Leadership, the Director of Ambulatory Quality and Population Health will lead and drive the development and implementation of quality improvement initiatives and population health management strategies across our ambulatory care network. This position will oversee clinical performance, quality metrics, patient outcomes, and population health initiatives to ensure the delivery of high-quality, equitable, evidence-based care to our diverse patient population.\n
The Director will collaborate closely with Leadership, operational teams, and community partners to optimize patient care and clinical workflows, improve health outcomes, and achieve value-based care objectives. This role requires a strong understanding of healthcare quality, data analytics, clinical operations, and population health management. This position has primary responsibility to ensure the organization meets annual quality regulatory reporting requirements (PCMH, MIPS, etc.) and ongoing compliance with quality measurement standards.\n

Roles and Responsibilities:\n\n\n
Strategic Leadership:\n\nWorking closely with leadership, work to develop, implement, and continuously evaluate the ambulatory care quality improvement (QI) and population health strategy. This includes collaborating with hospital\/facility counterparts for insure strategies span the continuum of care.\nPartner with Operational Leaders to define and drive initiatives to enhance patient outcomes and reduce health disparities using standardized process improvement methodologies.\nWorking closely with other key stakeholders, define and drive initiatives that optimize the efficiency of clinical workflows, for providers and other staff.\nOversee the development and monitoring of ambulatory quality measures, including adherence to clinical guidelines, patient satisfaction, and safety standards.\n\n
Population Health Management:\n\nDesign, implement, and monitor population health management programs aimed at improving care for high-risk and underserved patient populations.\nUtilize data analytics to identify gaps in care, manage chronic disease, and reduce unnecessary healthcare utilization.\nCollaborate with community organizations and stakeholders to address social determinants of health and improve patient access to care.\n\n
Quality Improvement and Performance Monitoring:\n\nMaintains expertise in quality improvement\/measurement, acting as the subject matter expert. This includes continuous monitoring of Centers for Medicaid and Medicare Services, NYS Department of Health, NCQA HEDIS\/QARR, NCQA PCMH etc.\nMonitor ambulatory care quality data, including performance on key indicators (e.g., HEDIS, MIPS, PCMH, ACO metrics, etc.), and ensure compliance with regulatory requirements.\nFacilitate the collection and analysis of performance data to identify opportunities for process improvements and implement corrective actions. This includes operational data (EMR utilization data) to help identify opportunities for efficiency with the EMR and educational needs.\nLead multidisciplinary teams to develop and implement best practice guidelines, clinical protocols, and other QI initiatives using standardized process improvement methodologies.\nIs flexible in assuming other appropriate responsibilities not noted above.\n\n\n\n
Required Skills and Experience:\n\n\n
Education - Master's degree in a healthcare related field\n\n\n
Experience - Minimum of 7-10 years of experience in healthcare quality management, population health, or ambulatory care leadership is required.\n\n\n
Applicant must have strong leadership and team management skills, with the ability to drive cross-functional collaboration and foster a culture of accountability, manage multiple priorities, interpret data, and design and implement plans of action using quality and process improvement techniques. Ability to navigate complex healthcare systems, develop innovative solutions, and effectively manage change.\n\n\n
Strong knowledge of current healthcare quality initiatives, experience with clinical quality metrics (HEDIS\/MIPS\/PCMH) including technical specifications. Experience implementing quality\/process improvement activities in the outpatient setting (e.g., Lean, Six Sigma, PDSA). Applicant should have excellent communication, presentation, and interpersonal skills, with the ability to engage and influence stakeholders at all levels of the organization.\n\n\n

Physical Requirements: \n\nMust be able to remain in a stationary position at least 50% of the time. The person in this position frequently communicates with Practices and Providers. Must be able to exchange accurate information in these situations. This position requires occasional driving to practice locations.\n\n

Preferred Skills and Experience:\n\nLicensure\/Certification - Quality management or process improvement certification preferred. Must complete CPHQ certification within 2 years of hire.\n\n

Pay Range Disclosure:\n\n$140,000 to $150,000 per year\n\n
