Director of Discharge Planning

Carolina Rehab Center of Cumberland in Fayetteville, North Carolina is looking for an experienced Director of Discharge Planning and Social Services to help residents and families prepare for their transition home. You will directly coordinate the patient's discharge including supplies, transportation, and other services to ensure a safe transition. We are looking for an energetic and dynamic self-starter with 2 or more years of experience in Discharge Planning and Social Services in a healthcare setting. The ideal candidate has a history of successful positions involving direct customer service, social services, coordination, and problem-solving, and teamwork. Strong communication and collaboration skills are a must.

 

Your day-to-day

Research, identify, and arrange external services for residents such as Chaplaincy, Advocates, Responsible Parties, and optimal care environments. Schedule discharge meetings and coordinate home visits, home care, therapy according to resident needs. Directs timely and efficient five-day discharge planning meetings for all newly admitted residents. Assesses patient's support systems and home situation to assist in decision-making regarding discharge plan and needs. Facilitates effective bed management by strategic evaluation and internal arrangement to maximize availability of transitional and extended care services. Meet with and assist residents with social issues such as conflict resolution, problem-solving, and care coordination. Conduct customer satisfaction surveys.

 

Qualified candidates will have a 4-year degree in Social Work, Psychology, Sociology, Education, Counseling, Criminal Justice, or other human services related degree and at least 1 year of Social Work experience in a healthcare setting.
