The Director of Nursing (DON) is a clinical manager who assists the Area Director of Clinical Operations (ADCO) and is responsible for the supervision/oversight of all patient care services and clinical personnel in an assigned location/branch of the Long Term Care (LTC) Division. The DON strives to provide the highest quality of patient care and oversight while achieving excellent outcomes in a patient-centered environment. In this role, you will report to the Area Director of Clinical Operations.
Supervisory Responsibilities:
Responsible for hiring, evaluating and terminating clinical personnel
Monitors equipment for appropriate use and takes steps to keep misuse to a minimum
Immediately investigates and resolves any accident, incident, lost articles, property loss, drug diversion, abuse/neglect and/or unusual occurrence(s) and initiates reports to the ADCO as necessary
Duties/Responsibilities:
Responsible for overall day to day clinical management of the LTC division branch ensuring supervision, competency, evaluation and planning for the clinical staff
Ensures oversight clinically of: making patient and personnel assignments, coordinating patient care, ensuring Patient’s Rights are adhered to, coordinating referrals, assuring that patient needs are continually assessed, and assuring the development, implementation, and updates of the individualized care plan(s)
Understands and adheres to all applicable regulations, accrediting standards, and federal, state, and local laws and established Agency policies and procedures
Available at all times during operating hours and participates in all activities relevant to the professional services furnished, including the development of qualifications and the assignment of personnel
Able to supervise, guide and develop patient care, personnel requirements, competency skills
Consults with the ADCO to determine a staffing pattern which will accomplish stated objectives and promote a maximum level of utilization of patient care staff while promoting a viable financial environment
Ensures that when applicable disciplinary action is necessary it is instituted, interpreted and documented. Responsible for all follow-up action on disciplinary action(s)
Communicates effectively with the Human Resources (HR) department
Responsible for the timely and complete orientation of new patient care personnel
Supports policies/procedures and uses appropriate channels for changes of such policies, if applicable
Responsible for the coordination of interdisciplinary team services and the documentation of care coordination
Ensures that all case conferencing efforts for patients are coordinated on a schedule addressing high volume, high risk and problem prone cases
Meets corporate goals for clinical metrics
Ensures completion of accurate and timely documentation and staff knowledge/completion of best practices according to Agency policy
Understands professional boundaries and ensures that clinical staff is trained in professional boundaries
Ensures that coordination of care and documentation occurs between staff and patient/family/caregiver /assigned representative and facility and other outside agencies
Ensures that the following information/instruction is given in writing to the patient/caregiver/representative: visit schedule with frequency of visits, complete Medication Profile, any treatments to be administered by agency staff (POC), any other pertinent instruction related to the patient’s care needs, and name and contact information of the agency’s Clinical Manager
Responsible for the organization of patient care services to delineate and delegate functional responsibility, lines of relationship and communication to provide safe and therapeutically effective service
Responsible for the coordination of Agency’s services with the services of other community agencies and documentation of that coordination
Observes confidentiality and safeguards all patient related information
Accepts responsibility for regular attendance and punctuality; fulfills job related requirements without regard to time involved
Serves as a resource person to clinical staff
Develops a cooperative relationship and communicates effectively and professionally with physicians and referral sources
Investigates, resolves and reports any problem relating to patient care and/or employee well- being
Attends pertinent continuing education programs as assigned by ADCO and shares information with staff
Responsible for the LTC branch ongoing Quality Assurance Performance Improvement (QAPI) program. Responsible for the identification of goals related to patient outcomes and coordinating, participating, and reporting of activities and outcomes results. Understands/ensures that all QAPI meetings and reports are collated and presented
Understands the EMS Plan and assists the Director of Operations in activating and documenting the plan
Functions as LTC branch infection control manager
Responsible for the review of patient medical records for compliance with federal, state and local laws, accrediting organization standards (if applicable), and Agency policies and guidelines
Assists Clinical Supervisor (CS) in conducting patient care conferences, staff meetings and ensures documentation
Ensures that all necessary supplies and equipment are available
Uses statistical data to determine quality and quantity/utilization of services
Participates in personal professional growth and development
Understands and promotes the goals of the Compliance Plan
Ensures Agency personnel have current clinical information and current practices
Must be able to perform nursing functions in times of staff shortage
Available/assists with staffing visit assignments and on-call
Participates in other administrative functions as assigned

Education/Experience/Licenses/Certifications:
Unencumbered professional registered nurse license in the state(s) in which practicing; Undergraduate degree (preferred)
Three years home health nursing experience (preferred)
At least two years healthcare experience in the field of supervision including one-year supervisory experience in home care or a closely related service (preferred)

Pay Range: $90,000 - $120,000 / salary with bonus
