The Neogen Care Director of Patient Care Services leads all home health care Clinical Operations while developing and growing Neogen Care home health care service capabilities. Responsible for planning, organizing, directing, and evaluating nursing, therapy, and health care services provided in the home setting and for the management of clinical operations. Establishes, implements, and evaluates processes, procedures, goals and objectives for Neogen Care to achieve and promote organization vision, mission, philosophy, and quality standards. The Director of Patient Care Services also serves as a Clinical Manager for the purposes of meeting Medicare and HMO client’s conditions of participation. ESSENTIAL FUNCTIONS: Under the direction of the CEO: + Coordinating and leading direct and indirect patient care services provided by Neogen Care  clinical personnel.  • Collaborating with the Director of Quality to support Neogen Care Quality Control operations,  processes, and functions.  • Leading, managing, mentoring, training, guiding, and counseling direct reports; specifically,  Clinical Supervisors.  • Developing, implementing, and evaluating clinical short and long-term goals and objectives;  focusing on clinical outcomes, value, quality, and experience.  • Taking measures to assure services are provided in compliance with all laws, regulations, and  policies and that services provided are highest quality, and managed effectively and efficiently.   • Promoting and achieving operational and financial performance through strategic planning,  implementation, and evaluation in collaboration with key operational partners.  • Ensuring quality patient care is consistently delivered following all local, state, and federal  guidelines.   • Establishing performance metrics and goals for clinical team members using measurable metrics  such as:  readmission rates, timely charting, missed visits, patient complaints, etc.    • Monitoring performance metrics and taking action to address performance shortcomings,  identifying any root causes or systemic causality if applicable.   • Supervising, coaching, and mentoring staff and participating in recruiting key clinical roles.  • Supporting a value-driven organizational culture promoting positive, highly engaged,  collaborative relationships among the various stakeholders.   • Provide guidance and counseling to Clinical Supervisors and clinicians.  • Assessing, planning, implementing, and evaluating patient and family/caregiver care for all  clinical personnel as indicated.  • Assisting the Chief Executive Officer in representing Neogen Care to external key stakeholders,  clients, and regulating bodies.  • Ensuring initial critical skills checks, 90-day evaluations, and annual clinical performance  evaluations are completed.  • Ensuring the proper and accurate maintenance of patient clinical records, statistics, reports, and  administrative records for purposes of evaluation and reporting of organization activities.  • Planning and implementing in-service and continuing education programs to meet education  and training needs of organization personnel.  • When required, to represent the CEO in an acting capacity.  • Other duties as assigned.  REQUIRED EDUCATION AND EXPERIENCE: + Registered Nurse (RN) with current license to practice in the State of California. + Undergraduate (BSN) degree in Nursing from a program accredited by the National League for Nursing. Graduate degree (health care) a plus. + Technically savvy - Able to comfortably use modern electronic systems including: EMR, IM system, Microsoft Office Suite applications, spreadsheets, and HR system supervisory roles. + Must have at least 10 years director or management experience. + Must have at least Five (5) years in a Home Health or Hospice related health care organization. + Knowledge and expertise in clinical decision-making for the patient population. + Demonstrated ability to effectively lead, supervise, and direct professional personnel. + Knowledge of business management, governmental regulations and regulatory standards. + Ability to work with high degree of precision and attention to detail to achieve accurate and timely delivery of services, and to follow up and close all open cases.
