Director of Social Services

Jefferson’s Ferry is a Life Plan community and a unique concept. It provides Independent Living and a comprehensive retirement living option for adults over the age of 62 by offering an Active Adult Community, Assisted Living, Skilled Nursing Care and Memory Care, all centrally located on one campus.

We are currently seeking a Director of Social Services to direct and supervise social service programs.

In this position, you will:

Evaluate the social needs of each resident admitted and formulate a written plan of care.Review and revise care plans.Refer residents and families to appropriate community resources.

We require:

Bachelors’ degree in social work; Master’s degree preferredExperience with Sigmacare and MDS completionLicense/Certification/Registration in accordance with New York state laws and regulationsKnowledge of Federal, state, and local regulations, particularly Medicare and Medicaid regulations and long-term care operational standardsExperience in a Skilled Nursing Facility required; assisted living experience preferredMinimum of three years of experience as a social worker with the geriatric population preferred
