Discharge Coordinator - Social Work (Call-In, Varied Shifts)
RESPONSIBILITIES I. JOB SUMMARY/RESPONSIBILITIES: • Plans and coordinates functions and activities related to meeting the psychological, social and nursing needs of patients and their families. • Supports the case manager and social worker with an emphasis on discharge planning and/or maintaining medically, frail individuals in the community. • Performs duties in accordance with all regulatory requirements as well as policies and procedures of The Queen’s Medical Center (QMC). II. TYPICAL PHYSICAL DEMANDS: • Essential: sitting, finger dexterity, seeing, hearing, speaking, frequent gripping of an object. • Continuous: static gripping of an object for prolonged periods. • Frequent: walking, lifting usual weight of 3 pounds; carrying usual weight of 2 pounds; reaching at shoulder level; repetitive arm/hand motions. • Occasional: standing, stooping/bending, kneeling, climbing stairs, walking on uneven ground, squatting, twisting body, lifting maximum of 10 pounds, reaching above and below shoulder level. • Operates calculator, computer, printer, telephone, and facsimile. III. TYPICAL WORKING CONDITIONS: • Not substantially subjected to adverse environmental conditions. • Subjected to noise and/or vibration due to shared office space, air conditioning and construction work in neighboring department. • May be exposed to hostile, disruptive, assaultive and inappropriate patient behavior. IV. MINIMUM QUALIFICATIONS: A. EDUCATION/CERTIFICATION AND LICENSURE: • Bachelor’s degree in social work or related field; or a bachelor’s degree in non-related field with two (2) years of experience working in a comparable job demonstrating knowledge of governmental and social agencies and services may be substituted for degree in social work or related field.
