Discharge Facilitator
**Overview** Works under the direction of the Manager for Case Management to provide support for discharge process. Performs follow up tasks identified by the discharge planners during the initial and ongoing phases of discharge planning for management of patients across the continuum. This position description has been designed to indicate the general nature and level of work performed by employees within this classification. Completes tasks related to transfer of patients to extended care facility including communicating with extended care facility related to bed availability, faxing of initial referral information, assuring transfer documentation completion, arranging transportation, confirmation of arrangements. communication with families of transfer arrangements. 2. Uses electronic and web based communication systems to communicate with outside agencies to facilitate transition planning. 3. Provides and educates families with “Medicare 2 nd Notice” letters as directed by Case Managers. 4. Notarizes advanced care planning documents upon request. 5. Provides list of facilities upon request to families/patients. 6. Acts as liaison to Medical Records for the obtaining of records to facilitate patient transitional planning. 7. Upon request of the discharge planner, mass faxes patient information to area facilities for bed offer. 8. Secures requested information for the healthcare team, patient/family. 9. Supports the Case Manager in tasks related to discharge to home/community including arranging transportation, confirming community arrangements, providing requested information via phone or fax. 10. Assists with special programs including data collection for process improvement projects. 11. Completes a variety of department specific tasks to assist with optimal department safety and functioning. 12. Appropriately communicates information in accordance with hospital policies and procedures. 13. Organizes and performs work effectively and efficiently. 14. Maintains and adjust schedule to enhance team performance. 15. Demonstrates positive interpersonal relations in dealing with all members of the team. 16. Communicates via electronic discharge system to obtain Medicaid authorization for patients discharging to SNFs. **Qualifications** + **Education:** High School Diploma or GED, required. + **Experience:** Health Care experience, Case Management or related experience, knowledge of Microsoft office products/keyboarding; preferred. + **Additional Skills Required:** Weekend and holiday coverage. Ability to travel between facilities. Excellent interpersonal and customer service skills. Working knowledge of personal computers and ability to learn a variety of software programs. Proficient spelling and grammatical skills. Attention to detail with excellent planning and organizational skills. Communicate effectively in both oral and written forms. Ability to operate standard office equipment including, but not limited to, a calculator, fax machine, copier, printer and scanner. Ability to successfully maintain effective work relationships.
