Experience Required

Identify and coordinate patient care needs in anticipation of the next site of care after hospitalizationProvide guidance for clinical coordination, and collaboration among multidisciplinary caregiversComplete comprehensive psychosocial assessment and review clinical needs of patients as they transition to the next level of careCollaborate with physicians, nurses, and the care team to foster a coordinated approach to discharge planningUtilize clinical knowledge to participate in treatment plan discussion with clinical teamsIdentify and connect patients to services and resources available to themCoordinates appropriate referrals to home care agencies, skilled nursing and rehabilitation centers, and community-based programsCoordinates care authorization process with insurersCommunicate with Utilization Review staff on any denials, issues, or barriers to discharge
Education Requirements

RN or BSW
License/Certification Requirements

RN or BSWLicensed in the State of NJ
Special Requirements

Minimum of 2 years’ experience in diverse clinical settings Licensed in the State of NJPrior case management /discharge planning experience preferredPrior experience with EPCI EMR and AllScripts, is preferred.Working knowledge of federal, state, and local laws that govern health care and case management.
