The **Discharge Planner** assists inmates/residents with medical or mental health issues in successful reentry into the community, ensuring continuity of care for chronically ill inmates/residents after discharge. This role communicates with external sources regarding inmates/residents on their caseload, including family contact.
+ Participates in the intake process
+ Collect, organize and analyze information about individuals through records, tests, interviews, and professional sources.
+ Responsible for establishing goals, developing plans, coordinating service delivery and appraising the interests, needs, aptitudes, abilities and personality characteristics of inmates/residents for effective reentry.
**Qualifications:**
+ Graduate from an accredited college or university with a Bachelor's degree in social work; or Registered Nurse (RN) with current RN licensure in the state of employment is required.
+ Three (3) years of discharge planning experience is preferred.
+ Experience in a prison/jail setting and/or in-patient medical facility is preferred.
+ Experience providing case management services is preferred.
+ Must demonstrate the ability to communicate clearly verbally and in writing.
+ Experience with Microsoft Office applications or other similar software applications is required.
+ A valid driver's license is required.
+ Minimum age requirement: Must be at least 18 years of age.
