The Discharge Planner assists inmates/residents with medical or mental health issues in successful reentry into the community, ensuring continuity of care for chronically ill inmates/residents after discharge.  This role communicates with external sources regarding inmates/residents on their caseload, including family contact. 

Participates in the intake processCollect, organize and analyze information about individuals through records, tests, interviews, and professional sources.  Responsible for establishing goals, developing plans, coordinating service delivery and appraising the interests, needs, aptitudes, abilities and personality characteristics of inmates/residents for effective reentry.

Qualifications: 

Graduate from an accredited college or university with a Bachelor's degree in social work; or Registered Nurse (RN) with current RN licensure in the state of employment is required.  Three (3) years of discharge planning experience is preferred.  Experience in a prison/jail setting and/or in-patient medical facility is preferred. Experience providing case management services is preferred. Must demonstrate the ability to communicate clearly verbally and in writing. Experience with Microsoft Office applications or other similar software applications is required. A valid driver's license is required. Minimum age requirement: Must be at least 18 years of age.
