Work directly with the hospital staff, other agencies, medical consenters, caregivers, and Department of Family and Protective Services (DFPS) to facilitate and coordinate access to behavioral health services and community resources and to ensure timely and clinically appropriate discharge plans are developed and implemented.
QualificationsEducation and Training Requirements:
Bachelor's Degree in Social Work required, Master's preferred.Texas State Licensed in Social WorkExperience: Two or more years of related social work and Case Management .Ability to read, write and communicate effectively in English.
