Discharge Planner - Emergency Department

Discharge Planner - Providence Regional Medical Center Emergency Department (Full Time, Day Shift)

Plans and coordinates activities related to transition planning.  Develops and implements a plan of care designed to meet the patient's post hospitalization needs while ensuring continuity of care.  Responsible for the collaboration with medical staff,  patient, family, community agencies, or facilities involved in patient's care, and the health care team by assessing, facilitating, planning and advocating for health needs on an individual basis resulting in quality, cost-effective outcomes.

Provides services to patients with social, mental, and cognitive impairments, per the risk stratification system.  Majority of work will be providing services to patients with complex needs for example: social history, family dynamics, psychological (ITA and Voluntary), medical complexity etc.

Required Qualifications:

Master's Degree in Social Work or Counseling Upon hire: Washington Clinical Independent Social Worker Associate License Or Upon hire: Washington Advanced Social Worker Associate License Or Upon hire: Washington Mental Health Counselor Associate License Or Upon hire: Washington Agency Affiliated Counselor Registration  Experience in discharge planning, care coordination, or community resource navigation, in a clinical or community setting. 

Preferred qualifications:

Experience in discharge planning for medical and or surgical patients in an inpatient or outpatient setting. Experience in an acute hospital setting doing discharge planning is preferred.
