**Summary:**
The Medication Assisted Treatment Discharge Planner assists inmates/residents with medical or mental health issues in successful reentry into the community, ensuring continuity of care for chronically ill inmates/residents after discharge. Communicates with external sources regarding inmates/residents on their caseload, including family contact.
**Essential Functions:**
+ The incumbent should be able to perform all of the following functions at a pace and level of performance consistent with the actual job performance requirements.
+ Participates in the intake process. Collects, organizes and analyzes information about individuals through records, tests, interviews, and professional sources. Establishes goals, develops plans, coordinates service delivery and appraise the interests, needs, aptitudes, abilities and personality characteristics of inmates/residents for effective reentry.
+ Confers with other clinical staff (e.g., psychiatrists, psychologists, mental health service providers, physicians, nurses, social workers, etc.) to gather and provide information concerning patient psychological issues, assesses discharge needs and identifies and implements methods for addressing/reducing these needs.
+ Creates a robust and sustainable release process for inmates/residents entering the community to include coordination of release planning measures with various stakeholders including classification committee, counseling services, community corrections, information technology, correctional health care vendors and community organizations that provide substance use disorder treatment services.
+ Meets with inmates/residents to discuss discharge needs, housing issues, applications for social services such as food stamps, Social Security disability, preparation for work and seeking employment, other available benefits and serves and/or other concerns.
+ Communicates with inmate/resident families in preparation for release.
+ Engages in community reentry services through discharge planning and communicates with others about service needs for releasing inmates/residents who participate in Medications for Opioid Use Disorder (MOUD) / Medication Assisted Treatment (MAT) programs. Schedules appointments for service providers to talk
+ with inmates/residents at the facility and for obtaining medications and medical/dental treatment. Assists with obtaining sources for alcohol and drug treatment.
+ Prepares progress reports and implements and maintains a dashboard for release planning on assigned inmates/residents. Provides oversight and guidance of data reporting for quality assurance. Assists administrative and supervisory staff in the development of statistical reports. Makes case presentations to administrative and supervisory staff on a scheduled basis.
+ Prepares and maintains a variety of standard narrative, statistical, summary and/or operational records, reports and logs to include but not limited to, documentation for transitions, transfers and releases. Maintains and monitors confidentiality of inmate/resident and administrative files. Processes all reports and documents in a timely manner.
+ Attends scheduled staff meetings, group meetings and other staff and group activities essential for effective program and service delivery. Promotes communications and the proper flow of relevant information between facility management, staff and inmates/residents.
+ Reads, analyzes, comprehends and interprets technical procedures or governmental regulations, legal and non-legal documents, including the processing of such documents as medical instructions, commitment orders, summons and other legal writs. Endeavors to comply with the requirements of applicable regulations, laws, rules, procedures, policies, standards and/or contracts.
+ Domestic U.S. travel may be required.
**Qualifications:**
+ Graduate from an accredited college or university with a Bachelor's degree in Sociology, Psychology, Social Work or Nursing is required.
+ If a Registered Nurse, must possess a current, active RN license in the state of employment.
+ Three years of discharge planning experience is preferred.
+ Experience in a prison/jail setting and/or in-patient medical facility is preferred.
+ Experience providing case management services is preferred.
+ Must demonstrate ability to communicate clearly verbally and in writing.
+ Proficiency with Microsoft Office applications or other similar software applications is required.
