Under the supervision of the Transportation Director/Designee, is responsible for the direct transportation of participants who may be physically or mentally limited to and from their destinations. Position requires constant interaction with participants, participant’s family and the staff of the Interdisciplinary Team.

Responsibilities

Job duties specific to this position:

Transports physically and mentally limited participants, door to door, to varied destinations both timely and safely. Bumps wheelchair participants up and down stairs or curbing as deemed appropriate by the Transportation Director/designee. Assists participants and their belongings on and off the van and to their destination in a safe and courteous manner. Assures that participants have appropriate clothing for weather conditions.Ensures that all participants wear safety belts and wheelchairs are locked and secured into position, following the policy and procedure for securing wheelchair. Assures at every stop that participant’s seatbelts and/or wheelchair securing is intact.Maintains the vehicle in a neat and clean condition. Cleans interior daily and washes outside of vehicle at least weekly. Follows infection control guidelines as appropriate. Fuels vehicle daily.Inspects vehicle daily to ensure the vehicle is prepared for safe, legal operation prior to departing. Immediately reports to the Transportation Director/designee any vehicle operating problems, malfunctions, or incidents out of the ordinary. Participates in preventative maintenance activities. Assists DME department as requested in cleaning and delivery of equipment and supplies.Works with Day Center Manager and clinic to coordinate delivery of supplies and medications.Identifies and reports any unusual circumstances regarding participant’s status to Transportation Supervisor or Transportation Director.Drives vehicle in a safe manner. Complies with all New York State Vehicle and Traffic laws. Completes all required vehicle and departmental paperwork. Completes all requested Quality Assurance forms and studies.Attends staff meeting and training as required.In the event of an incident or accident, reports immediately to a supervisor. Collects all required information and completes incident/accident reports in accordance with company policy and under Transportation Director/designee’s direction.Participates in the maintenance of a safe, clean, and healthful transportation environment.Other duties as assigned, including but not limited to, assisting with PACE center activities and escorting participants within the buildings.Practices safety awareness, reports any possible safety/environment issues to supervisor, and adheres to safety policies and procedures. Displays compliance oriented behavior in the workplace. Is responsible for promoting and fostering compliance in the workplace. Adheres to service excellence by developing and maintaining positive respectful relationships will all customers, internal and external, to include participants, families, team members, and all staff throughout the organization and community partners; performs responsibilities according to the highest quality standards.Demonstrates knowledge of PACE/D&TC Emergency Preparedness Plan and care verbalize their role during an event.If working as a Per Diem Driver, is available to work during Loretto prime time, and other peak vacation periods, as well as days prior to and following paid holidays.

Qualifications High School Diploma, GED or ability to pass a written TABE test.Current valid New York State driver’s license, free from major infractions and acceptable by our insurance carrier.Must participate in the NYS LENS program (License Event Notification) and remain compliant throughout employment.Completion of approved course in First Aid within three (3) months of hire date.At least one (1) year experience transporting individuals with special mobility needs.Demonstrated communication skills.Ability to multi-task. Ability to follow written and verbal directions.Excellent customer service skills.

PHYSICAL REQUIREMENTS

Vision sufficient to maintain a driver’s license. Ability to distinguish color.Must be able to hear and verbally communicate.Ability to remain alert at all times for duration of shift. Full range of motion including lifting, bending, and reaching with full mobility for work with elderly persons. Must be able to lift, push, pull and move a minimum of 50 lbs. Must be able to assist in moving participant as needed. Manual dexterity sufficient for assisting elderly with daily functions.
